G2211 update: CMS will allow payment for evaluation and management visits with added code. It can now be paid when billed with a base E/M code, on the same day as an annual wellness visit, vaccine administration, or any Medicare Part B preventive services. This was initially prohibited as the G2211 code was not allowed to be paid on any claim that used modifier 25.
G0537: This risk assessment code would cover the administration of an ASCVD risk assessment tool, for patients without cardiovascular diseases but with risk factors, e.g. high blood pressure, obesity, diabetes, or smoking history.
G0538: This code will cover the risk assessment factors for the patients at intermediate, medium, or high-risk factors, including medication history, blood pressure, cholesterol management, and so on.
Modifier 54: From now on this modifier will be required for 90 global surgical packages, when a practitioner performs only the surgical portion. A formal transfer of care agreement is not needed in this case. Payment will be adjusted to exclude follow-up care.
G0559: This new added-on code will be used for postoperative care provided during a 90-day care period, by a different specialty or practitioner. Just like the modifier 54, this code also does not require any formal transfer.
